Service Information

Pet Health & Safety Questions (please select Yes or No)

Has your pet received any vaccinations in the last 48 hours?
Has your pet been given any sedatives or medications today?
Has your pet been treated for fleas/ticks in the past 30 days?
Does your pet have any medical conditions we should be aware of? (i.e heart issues, joint issues, skin tags/sores)
Does anything make your pet anxious or nervous? (e.g., nail trimming, dryers, etc.)

Service Agreement & Terms

By signing this agreement, you acknowledge and agree to the following:

  1. Grooming Process & Duration

    • Grooming times vary depending on your pet's size, coat, condition, and temperament.

    • Most sessions take up to 2 hours, but may exceed this if additional time is required.

  2. Security Deposit

    • A non-refundable deposit ($50) is required to secure your appointment.

    • This amount will be applied to your final balance unless a cancellation occurs within 24 hours or a no-show.

  3. Cancellations & No-Shows

    • I agree to provide at least 24 hours’ notice for cancellations or rescheduling.

    • I understand that canceling within 24 hours may result in a 50% cancellation fee, and no-shows will be charged the full grooming price.

  4. Health & Vaccination Policy

    • I confirm my pet is in good health and up to date on all required vaccinations (Rabies Vaccine).

    • I understand that grooming will not be performed within 24 hours of my pet receiving vaccinations due to risk of soreness or adverse reaction.

  5. Matted Coat Disclosure

    • I understand that matted pets may require shaving or de-matting at an additional charge.

    • I acknowledge that grooming matted coats may increase risk of skin irritation, nicks, or redness, and I will not hold the groomer liable for these outcomes.

  6. Fleas & Ticks

    • I understand that if my pet is found with fleas or ticks, they will receive a mandatory flea bath at an additional cost, and I may be charged a cleaning fee if infestation is severe.

  7. Behavior & Temperament

    • I have disclosed any behavioral issues such as aggression, anxiety, or bite history.

    • I understand that extra time and handling for behavioral concerns may incur an additional special handling fee.

    • If my pet poses a danger to the groomer or themselves, services may be stopped and a partial fee will be charged.

  8. Senior Pets & Puppies

    • I understand that elderly pets and young puppies may require special handling and that certain services may be limited based on their health, behavior or stress level.

  9. Photos & Social Media

    • For the protection of both your pet and our business, we routinely take before-and-after photos of all grooming appointments for documentation and safety purposes.

    • With your permission, we may use these photos for marketing purposes, such as posting on our website or social media platforms (e.g., Instagram, Facebook).

    • Photos used for promotional purposes will never include your personal information.

    • If you prefer that your pet’s photos not be used for public or promotional content, please indicate below.

Do you give permission for Purrfect Pawradise LLC to use photos of your pet for marketing/social media purposes?

  1. Liability Waiver

    • I release Purrfect Pawradise LLC, its owner, and employees from any and all liability for injury, illness, or death of my pet during or after grooming, unless caused by proven negligence.

    • I accept full responsibility for damages or injuries caused by my pet to people, property, or equipment.

By signing below, I confirm that I have read, understand, and agree to the terms of this contract and to abide by all grooming policies set by Purrfect Pawradise LLC.

🔍 Understanding Matting and Its Risks

Your pet has been presented with one or more areas of matting—dense tangles and knots of fur that can cause discomfort, skin irritation, and other health issues. Matting may trap moisture, bacteria, and parasites, and can lead to:

  • Skin bruising or abrasions

  • Hidden wounds or infections

  • Restricted movement or circulation

  • Increased sensitivity and pain during grooming

  • Behavior changes due to discomfort or pain



✂️ Dematting and Shaving Policy

Depending on the severity of matting, the only humane option may be to shave the coat completely using close blades to the skin. This is to avoid pain and trauma from excessive brushing or dematting attempts.

Please understand:

  • Shaving may expose pre-existing conditions such as sores, rashes, or parasites.

  • Your pet’s skin may be sensitive or nicked despite the utmost care.

  • Shaving a matted coat is not a cosmetic groom—it is a necessary procedure for the pet’s health.

  • Regrowth may be patchy or have a different texture.

I give permission to the groomer to shave my pet if deemed necessary and in the best interest of the animal’s well-being.


I understand that there is a risk of skin irritation, clipper burn, or nicks when shaving a matted pet, and I will not hold the groomer or business liable.


I acknowledge that the groomer may find underlying health conditions once mats are removed, and they are not responsible for pre-existing conditions.


I understand that excessive matting may incur an additional fee due to time and equipment wear.
